Arteriovenous malformations of subcortical area: clinical manifestations and results of endovascular treatment

Abstract

Objective — to study clinical manifestations; to evaluate the efficiency and safety of endovascular treatment in patients with subcortical brain arteriovenous malformations.
Materials and methods. It was analyzed clinical manifestations of 31 patients with subcortical brain arteriovenous malformations. There were 16 (51.6 %) men and 15 (48.4 %) women. The age of the patients was from 9 to 53 years (mean age of patients was 26.7 years). It was evaluated the results of treatment of 27 patients, who underwent surgical intervention, according to data of the physical and neurological examination, selective digital subtraction angiography, computer and magnetic resonance imaging.
Results. Clinical manifestations reveled that 28 (90.4 %) patients had intracranial hemorrhage, 2 (6.4 %) — headache and 1 (3.2 %) — focal neurological deficiency. The volume of malformations obliteration after treatment: total obliteration was achieved in 17 % patients, subtotal — in 56 % and partial — in 37 %. New focal neurological deficits in early postoperative period was observed in 9 (33.3 %) cases, however it regressed at the time of discharge in 7 (77.8 %) patients. There weren't hemorrhagic complications as well as mortality cases after the treatment.
Conclusions. Endovascular surgery is an effective and relatively safe method of treatment of brain arteriovenous malformations, which is used as an independent method or part of combined therapy. That allows to achieve favorable results in early and late postoperative period.
